Contact the Hospice Nurse who will respond to your home and make the pronouncement of death.

and Medical Examiner or Law Enforcement is involved.

Authorize the Medical Examiner to release the loved one to Miramar Memorial Services - 858-824-1908.
Once written authorization has been obtained, our staff will transfer your loved one into our care.

Make arrangements to transfer your loved one to our facility. You can rest assured that your loved one will be well cared for by our dedicated and experienced professionals.
Schedule an appointment so we may complete the paperwork for you and and have the legal documents signed by the appropriate parties.
Ask you to print this form to prepare for the appointment. There may be information you do not know. Use the word "Unknown" in those particular fields.

Depending on the doctor or coroner signing the death certificate, and our crematory schedule, we will contact you once the cremated remains are ready in approximately two weeks.

If you are ordering certified copies of the death certificate, they will available in approximately 2-3 weeks.
